Instructions:
1) Print out this form and fill in how many of each type of T-shirt, Sweatshirt or Hooded Sweatshirt you would like to purchase.
2) Specify the size(s) you need in the Size box for that item.  Example:  4 Adult Smalls and 1 Youth Medium, etc.
3) Total up the Item (Quantity of Items x Price) noting whether you qualify for the SOSHB member discount and then add all the Item costs in the Sub Total Box.
4) Add Shipping and Handling if we are mailing this to you:  $5.00 per item.  If you wish to pick up the T-Shirts from SOSHB, leave out this amount and call Karen Chepeka at (714) 442-1446 to arrange your pickup.
5) Add it all up for a Grand Total make Check or Money order out to:
